Title:Interferon-Gamma responses to candidate leprosy skin-test reagents detect exposure to leprosy in an endemic population.

Abstract:New tools for the detection of leprosy exposure in a community will be necessary for the eradication of leprosy. Candidate leprosy skin-test antigens derived from the fractionation of the leprosy bacillus into cytoplasmic and cell-wall proteins free of immuno-inhibitory mycobacterial lipoglycans and carbohydrates were used in an overnight blood test to determine whether exposure to leprosy can be detected by the production of the cytokine interferon gamma (IFN-gamma). Strong IFN-gamma responses were detected in leprosy contacts to both skin-test antigens compared with control subjects from the same endemic communities. There was little response in patients with tuberculosis. Responses were greatest in contacts with recent leprosy exposure. The implications of these findings for the application of these reagents in a field trial as skin tests to detect exposure to leprosy are discussed in light of the strong association between overnight IFN-gamma to PPD and the tuberculin skin-test responses previously reported. Abstract:The antigenicity of Mycobacterium leprae (M. leprae)-derived cell membrane fraction was examined using human dendritic cells (DCs). Immature DCs internalized and processed the cell membrane components, and expressed M. leprae-derived antigens (Ags) on their surface. The expression of MHC class II, CD86, and CD83 Ags on DCs and CD40 ligand (L)-associated IL-12 p70 production from DCs were up-regulated by the membrane Ags. Moreover these stimulated DCs induced significantly higher level of interferon-gamma (IFN-gamma) production by autologous CD4(+) and CD8(+) T cells than those pulsed with equivalent doses of live M. leprae or its cytosol fraction. Both subsets of T cells from tuberculoid leprosy patients also produced several fold more IFN-gamma than those from normal individuals. Furthermore, the intracellular perforin production in CD8(+) T cells was up-regulated in an Ag-dose dependent manner. These results suggest that M. leprae membrane Ags might be useful as the vaccinating agents against leprosy. (AU).

Abstract:La existencia de un defecto específico en la respuesta inmune mediada por células ha sido señalada en la lepra lepromatosa. Se sugiere la posibilidad de uq el interferon (IFN) gamma influya em el dominio del espectro inmunológico lepromatoso. Se hace un estudio para demostrar si el IFN gamma como coadyuvante de la terapéutica específica es capaz de transformar o modificar el efecto inmunológico en la lepra lepromatosa. Se seleccionarán 20 pacientes vírgenes de tratamiento o con menos de dos años de terapéutica específicas distribuidos de acuerdo a tabla aleatoria en dos grupos que reciben IFN gamma o placebo además de la quimioterapia especifica. Antes del tratamiento y al final del mismo se realizarán los siguientes estudios: histopatológico, inmunológico y bacteriológico. Se presenta comunicación preliminar de acuerdo a corte realizado en los dos pacientes que culminaron su estudio: uno tratado con IFN gamma y otro que recibió palcebo, siendo la respuesta notablemente superior en el primero desde los puntos de vista clínico, bacteriológico e inmunológico, llegando a negativizar la detección de bacilos. Abstract:A diverse range of infectious organisms, including mycobacteria, have been reported to induce cell death in vivo and in vitro. Although morphological features of apoptosis have been identified in leprosy lesions, it has not yet been determined whether Mycobacterium leprae modulates programmed cell death. For that purpose, peripheral blood mononuclear cells obtained from leprosy patients were stimulated with different concentrations of this pathogen. Following analysis by flow cytometry on 7AAD/CD14+ cells, it was observed that M. leprae induced apoptosis of monocyte-derived macrophages in a dose-dependent manner in both leprosy patients and healthy individuals, but still with lower efficiency as compared to M. tuberculosis. Expression of tumour necrosis factor-alpha (TNF-alpha), Bax-alpha, Bak mRNA and TNF-alpha protein was also detected in these cultures; in addition, an enhancement in the rate of apoptotic cells (and of TNF-alpha release) was noted when interferon-gamma was added to the wells. On the other hand, incubation of the cells with pentoxifylline impaired mycobacterium-induced cell death, the secretion of TNF-alpha, and gene expression in vitro. In addition, diminished bacterial entry decreased both TNF-alpha levels and the death of CD14+ cells, albeit to a different extent. When investigating leprosy reactions, an enhanced rate of spontaneous apoptosis was detected as compared to the unreactive lepromatous patients. The results demonstrated that M. leprae can lead to apoptosis of macrophages through a mechanism that could be at least partially related to the expression of pro-apoptotic members of the Bcl-2 protein family and of TNF-alpha. Moreover, while phagocytosis may be necessary, it seems not to be crucial to the induction of cell death by the mycobacteria. (AU).

Abstract:Antigen and mitogen-induced gamma interferon (gamma-IFN) production was studied in peripheral blood mononuclear cells from 34 leprosy patients. 17 of 18 lepromatous leprosy and borderline lepromatous patients (LL and BL) failed to release gamma-IFN in response to specific antigen (Mycobacterium leprae) and displayed reduced responses to mitogen (concanavalin A) stimulation. In contrast, cells from six tuberculoid and borderline tuberculoid patients (TT and BT) produced considerable levels of gamma-IFN under the same experimental conditions. Normal controls failed to respond to M. leprae and most displayed good responses to concanavalin A. Mid-borderline patients (BB) showed intermediate levels of gamma-IFN release. gamma-IFN release by lepromatous patients could be partially restored with purified interleukin 2 and M. leprae antigen but not with interleukin 2 alone.(AU).

Abstract:A hanseníase é uma doença espectral que apresenta grande variação no quadro clínico, diretamente relacionada com a resposta imune celular do hospedeiro dirigida contra os antígenos do Mycobacterium Leprae. Assim, no pólo tuberculóide, o quadro clínico de poucas lesões associa-se a uma baixa carga bacilar e à presença da resposta imune celular contra o bacilo de Hansen. No outro pólo da doença, a forma virchowiana, o quadro clínico de lesões disseminadas está associado a uma alta bacilífera e ausência de resposta imune celular. A produção de citocinas pelo sistema imune parece de fundamental importância na regulação da resposta imune humoral e celular, sendo por sua vez modulada por antígenos micobacterianos diversos. Portanto, as duas formas 'polares da hanseníase devem se diferenciar quanto ao perfil de produção de citocinas em resposta ao M. leprae, e difrentes antígenos micobacterianos devem modificar este perfil.(AU).
